Should I be concerned with purchasing a diamond with an older GIA certificate 5/17/99...and in the comments section it says: Additional clouds are not shown. Additional extra facets are not shown.

Those comments don't bother me, however you may want to make sure that you get the stone checked by an independent appraiser to make sure the grade is still accurate. It could be a stone that has been used and returned several times. A stone being worn over time could get some chips on the girdle or scratches lowering the clarity grade.
